What to expect, step by step

  1. 1

    Call to schedule

Call 516-325-1965 to schedule. Have ready your full name, date of birth, the last four digits of your Social Security number, and your NYS driver’s license number. Please don’t email Social Security or license numbers — we’ll take them on the call.

  2. 2

    Complete intake paperwork

We email your intake paperwork and an IDS consent form to review and return before your appointment.

  3. 3

    Attend your assessment

During the evaluation we discuss the incident, drinking and substance use history, family and work factors, and relevant medical and legal history.

  4. 4

    Complete a toxicology test

You complete a required toxicology test at a facility of your choice. That testing is done outside our office and isn’t included in the $725 fee.

  5. 5

    Receive your results

If no further care is recommended, results can be uploaded to the NYS DMV/OASAS system within days, sometimes the same day. Your attorney is notified if requested.

A thorough, confidential evaluation

The evaluation determines whether there's evidence of substance misuse and any risk of a repeat offense, with a referral to the appropriate level of treatment only if it's genuinely needed. We emphasize flexible scheduling and personalized, confidential care throughout.
